For revisions I do charge by the hours - using a quarter-hour minimum fee.
And yes, I do charge to put documents to diskette for clients. And, due to the potential of diskettes introducing viruses to my system, I ONLY use diskettes I provide. I usually charge $5.00 - which covers the diskette and the time/hassle factor.
And, FYI regarding your charge for the original typing you stated:

If those 9 pages were single-spaced lines with 1" margins all around you must type allot faster than I do to make that fee worthwhile. Last I remember (and it's been a while since I've researched it), industry standard fees for double-spaced page with one-inch margins was $3.00. And that's assuming clear, crisp readable copy - straight typing - not allot of numbers or industry specific terms.
